If you wish to request for a dental report, please note that you will be required to give official consent to the release of your information. For patients below 21 years of age, the consent of patient’s parent or legal guardian is required.

 

If you are applying for the dental report of a patient who has since passed away, a copy of the death certificate and the consent from the deceased’s spouse are required.

How do I apply for a dental report?

 

You may either

 

• Mail a completed copy of the Clinical Abstract application form, together with your cheque payment, to the address listed below
• Visit us at the Central Records Office, Level One, National Dental Centre of Singapore. We open Mondays to Fridays, 8.00am to  5.15pm.
        

 

How do I make the payment ?

   

• Payment can be made using cash, credit card, cheque or NETS at the Central Records Office.
• Cheques should be crossed, made payable to the ‘ NATIONAL DENTAL CENTRE OF SINGAPORE PTE LTD’ and mailed to:

             (Dental Report)

             5, Second Hospital Avenue
             Singapore 168938

Please quote the name and NRIC of the patient on the back of the cheque.  Please do not send in any cash by post.

 

 

How long does it take to process a Dental Report request?

  

Processing time for the reports is between 3 to 4 weeks.

 

 

What do I need to bring when collecting the report?

 

Please bring along your NRIC when collecting the report.

 

An authorisation letter stating the collector’s name and NRIC is required if he/she is collecting the report for you.

 

If the patient is deceased, please bring along a copy of the death certificate.
